H4 → J1 physician change of status with spouse on H1B — anyone in a similar situation?

Hi everyone! Looking for experiences from anyone who has been in a similar situation. I’m currently in the US on H4, and my spouse is working here on H1B. I’m applying for residency and, if I match at a J1-sponsoring program, I’m considering doing an H4 → J1 physician change of status within the US rather than going to India for J1 stamping. My main concern is 214(b)/nonimmigrant intent because my spouse is already living and working in the US on H1B. Has anyone here: Changed from H4 → J1 physician while their spouse was on H1B? Later traveled back home and successfully got their J1 visa stamped? Been questioned about their spouse being on H1B or had issues proving ties back home? Received a 214(b) refusal in this situation? Would really appreciate first-hand experiences.
