How do I climb out of this hole

I gotta start out the gate with I have nothing I can put down and terrible credit. I can't afford my current car payments. It was all I could get approved for at the time and I was desperate, but I'm constantly behind. I have 32k as the payoff amount from the bank, with a car worth 19k. I have an equinox right now that I purchased before my son was born and we didn't know he'd be special needs. I really need a minivan to accommodate for my son's equipment. I know most banks won't want to even look at me. But I can't keep going with the current monthly payments, I've gotta get them lower and find a different vehicle. Am I being delusional in hoping I have any option at all out there?
